LCP Transportation provider guide: joining its Indiana Medicaid network

To become an LCP Transportation provider, enroll with Indiana Medicaid (IHCP) first, under the transportation provider type. Next, send LCP its Transportation Team Application and the documents on its New Vendor Credentialing Checklist. LCP is an Indianapolis broker, founded in 1998, that arranges rides for UnitedHealthcare Community Plan's Hoosier Care Connect and PathWays for Aging members, and for Humana PathWays members.

Who LCP Transportation is

LCP Transportation, LLC is an Indianapolis company that has managed non-emergency rides since 1998. It works for Medicaid managed care plans as a broker: members call its statewide call center, and LCP passes trips to vendors in its network. It also runs its own drivers alongside those vendors, for regular and urgent trips in the denser urban areas and for some after-hours and long rural trips where 24-hour local service is scarce.

LCP runs its own proprietary software. The system, which it calls MILE (Managed Integrated Logistic Exchange), handles scheduling, automatic assignment of trips to vendors, automatic dispatch of return rides, electronic manifests, trip reporting, and vendor payments. LCP also processes emergency ambulance claims for managed care plans.

For vendors, the useful part is how LCP describes its network. It prefers local companies that know the traffic and the communities in their area, and it checks them through periodic vehicle inspections and on-site visits.

Which members LCP serves

Indiana splits Medicaid rides by program and plan. LCP covers two plans.

PathWays for Aging launched on July 1, 2024, and its members get rides through their plan. So a fleet that wants every Indiana Medicaid trip in its area needs three contracts: LCP, WellTrans, and Verida. The Indiana NEMT guide covers each program.

On the member side, LCP asks for two business days’ notice and allows one escort. Humana tells its PathWays members to book at least 48 hours ahead and to cancel at least 24 hours before the appointment.

Before you apply: Indiana enrollment and authority

LCP’s checklist starts with proof that the state has already enrolled you. Plan on two state approvals before LCP can credential you.

IHCP enrollment as provider type 26

Indiana Health Coverage Programs (IHCP) enrolls non-ambulance ride companies as provider type 26. The company has to serve the general public, and carrying passengers has to be its primary business.

A few rules apply to all three:

  • Surety bond. $50,000 or more, with a term of three years or longer. A 501(c)(3) nonprofit is exempt, as are companies owned by a licensed hospital or pharmacy. FSSA may also waive the bond for a provider serving an underserved area.
  • Driver details. For every driver: full name, date of birth, and Social Security number, plus the provider’s signed attestations.
  • Nonprofit insurance. A nonprofit carrier must carry commercial auto liability with a combined single limit of $1.5 million or more.
  • Risk level. Taxi and common carrier enrollments count as high risk the first time, which brings an application fee ($750 for enrollments received in 2026), an unannounced site visit, and fingerprint-based background checks. At revalidation they drop to moderate. See provider risk levels.

State operating authority

Under IC 8-2.1-22, the Indiana Department of Revenue issues a Certificate of Public Convenience and Necessity to every for-hire passenger carrier. The application fee is $100. Review runs 30 to 60 days, and no paid passengers may ride while it is pending. For vehicles carrying 15 or fewer people, the required commercial liability is $1.5 million. After approval, the company gets 60 days to file proof of insurance (a Form E from your insurer) and its tariff. The authority renews by November 30 every year.

That certificate, or a taxi license, is the “transportation authority” item on LCP’s checklist.

How to apply to LCP

LCP takes applications online.

  1. Read the checklist. Download the New Vendor Credentialing Checklist from LCP’s provider network page so you know what comes next.
  2. Submit the Transportation Team Application. The form asks for your company name, contact person, address, phone, cell, fax, and email. It counts your vehicles by type: passenger (capacity 4), van (capacity 6), lift (capacity 2), and stretcher (capacity 1). It also asks for your home county, the other counties you serve, and whether you travel out of state.
  3. Send the credentialing file. LCP’s vendor relations team collects the documents below. Questions go to vendorsupport@lcptransportation.com.
  4. Sign the NEMT Agreement. LCP sends it for you to initial and sign. Confirm your rates and the payment schedule before you do.

The checklist was last updated July 7, 2026. Group it into three piles as you build the file with the broker credentialing checklist.

  • IHCP enrollment letter and IHCP provider profile
  • The signed NEMT Agreement
  • An ACORD certificate of insurance naming LCP Transportation, LLC as additional insured, with the required wording in the Description of Operations box: LCP is named as an additional insured, and 30 days’ notice of cancellation will be given for any reason, including nonpayment of premiums
  • A workers’ compensation clearance certificate, if workers’ compensation is not on your policy
  • Your transportation authority: the Certificate of Public Convenience and Necessity, another transportation authority certificate, or a taxi license
  • Vendor profile sheet, Disclosure of Ownership, W-9, and direct deposit form

Timing matters with the six-month rule. Order the criminal checks, drug tests, and MVRs together, close to the day you submit, so none of them ages out while you gather the rest. The driver background check guide covers what each report shows.

Working with LCP

Once you are active, trips and paperwork run through LCP’s vendor portal. There you see your routes and passenger pickup times, pull schedules, and find invoices.

LCP captures most trip details when the member books. The rest comes from the manifest that you or your driver submit. LCP combines the two into the completed trip record it sends to the health plan, so a gap in your manifest becomes a gap in that record.

A few habits keep the relationship healthy:

  • Report no-shows right away. The vendor and driver line, (765) 566-7204, handles no-shows and courtesy calls.
  • Expect grouped dialysis runs. LCP says it coordinates schedules with patients and dialysis centers so minivans can combine trips.
  • Keep vehicles inspection-ready. LCP checks vendors with periodic vehicle and on-site inspections.

LCP also tracks members with repeated no-shows, cancellations, or unusual trip patterns, and it confirms scheduled appointments, which it says cuts down on fraudulently booked trips.

Keeping credentials current

A broker file is full of dates: licenses, insurance, CPR cards, and training certificates all expire. HealthRide tracks the expiration date on each driver and vehicle credential and sends reminders before it runs out. If a credential has lapsed, HealthRide warns you when you try to assign that driver or van. Those dates live with your drivers and vans in fleet.
