How adult day transportation works
Adult day centers give seniors and adults with disabilities a supervised place to spend the day while caregivers work or rest. CDC’s National Center for Health Statistics counted 3,100 adult day services centers in 2022, serving 197,700 participants on any given day. Participants who can’t drive themselves need a ride in the morning and a ride home in the afternoon.
The center may be the payer. In New York, many adult day health care programs either own vehicles or contract with Medicaid-enrolled transportation providers, and the program pays the provider directly rather than Medicaid. In Virginia, Medicaid treats rides to day support programs as standing orders when the member attends at least one day a week for 90 days. Waiver enrollees ride often: CMS found that 29 percent of section 1915(c) waiver enrollees used NEMT in 2019, averaging 3.5 ride days a month.
For a transportation provider, a center contract means the same riders, the same addresses, and the same two time windows, five days a week.
Where adult day routes break down
Adult day work looks simple on paper. It gets complicated one participant at a time.
- Everyone arrives at once. Participants need to reach the center close to opening time, and most leave near closing time. The morning and afternoon windows are where the day is won or lost.
- Attendance days differ by participant. One attends Monday, Wednesday, and Friday. Another attends Tuesday and Thursday. A third just switched from three days to five.
- Absences come in late. A caregiver calls at 6:45 AM to say their mother is sick. If that change doesn’t reach the driver, the van sits in a driveway and the next pickup runs late.
- Early pickups happen. A participant feels unwell at 11 AM and the center calls for a ride home.
- Drop-off needs a person. Participants with memory loss may not be safe at an empty house. Drivers need to know who must be home to receive them, which is what hand-to-hand service means.
- The center’s records aren’t yours. New York lists an attendance log from a day program as supplemental documentation only. It can’t be the only proof that a trip happened.
- The invoice has to match. At month’s end, the center compares your invoice against attendance. Every skipped day that still shows as a trip is a dispute.
