How to become a Modivcare transportation provider in 2026
To become a Modivcare transportation provider, send the interest form on its transportation provider page, then pass credentialing. Drivers need PASS, CPR, defensive driving, a drug screen, a driving record, and a background check. Vehicles need current registration and inspection. You also carry general liability, sexual abuse coverage, and auto insurance, sign a Transportation Agreement, and bill trips against the check run dates in its provider portal.

Who Modivcare is
Modivcare calls itself the largest NEMT broker in the country, with more than 6,500 subcontracted transportation providers and about 35 million paid trips a year. State Medicaid agencies and health plans hire it to run their ride benefits. It also has personal care and remote patient monitoring businesses. It does not run the rides itself. It contracts local transportation companies, assigns them trips, and pays them.
The transportation business used to be called LogistiCare. Today the NEMT contracts sit with ModivCare Solutions, LLC. Its parent is Modivcare Topco, LLC. The company is based in Denver.
The company went through Chapter 11 in 2025. It filed on August 20, 2025 in the Southern District of Texas. It emerged on December 29, 2025 with about $1.1 billion less funded debt, more than 85 percent of what it owed, plus $100 million in new capital. The old stock was canceled and the company left SEC reporting. It now runs as a private company owned by the creditors and investors who received its new equity. Modivcare says members, clients, and providers saw no interruption during the case.
Where Modivcare operates in 2026
Modivcare holds statewide Medicaid contracts in several states and health plan contracts in others. These are its main Medicaid programs in 2026. Plans change vendors often, so confirm the member’s plan before you plan capacity around it.

How to become a Modivcare transportation provider
Modivcare’s partner relationship managers answer provider questions along the way. Expect these steps.
- Check that Modivcare holds your market. Use the table above and the member plans in your area. Modivcare’s provider page also shows a map of the areas where it needs providers most.
- Send the interest form. It sits on the Become a Transportation Provider page and starts the conversation about your state’s contract.
- Credential the company, drivers, and vehicles. Modivcare tracks credentials in Complicore. Every driver’s name and license number, and every vehicle, must match what is on file there, or claims get denied later. Our broker credentialing guide covers how to build the file once and reuse it.
- Pass the field visit. Modivcare staff visit providers and check each vehicle against the contract.
- Sign the Transportation Agreement. It sets your service area, rates, and the number of days you have to bill a trip.
- Complete compliance training. Owners and drivers review the code of conduct, compliance training, and billing tips, then sign an attestation in Complicore. New providers do this at contracting and every year after.

Modivcare driver, vehicle, and insurance requirements
Modivcare’s national checklist is short. State contracts add detail, so ask your relationship manager for the exact limits in your market.

How trips reach you
Modivcare assigns trips to contracted providers. It says strong performers get more volume, including extra trips that open up from cancellations, no-shows, and long waits.
Modivcare offers WellRyde, its dispatch platform, at no or reduced cost for Modivcare trips. It also works with other dispatch systems, so you can keep your own software.
Trip rules come from each state contract. Virginia is a useful example:
- Hospital discharges. Modivcare has three hours from the hospital’s call to assign the trip and pick up the member.
- Late pickups. A driver more than 15 minutes late leads to a Provider Late complaint through the Ride Assist line.
- Will-call returns. Once the member or facility calls Ride Assist, the provider has up to 45 minutes to arrive. See will-call trips.
- Mileage. Pay is based on the shortest street or road distance. Raise a mileage error before you run the trip, because running it means you accept the mileage.
- Late cancellations. If a member does not cancel at least the day before, the provider may arrive and not be paid. The provider can file a Rider No-Show complaint instead.
How Modivcare pays providers
Modivcare pays the rates in your Transportation Agreement. Rates are set contract by contract, so read the rate schedule before you sign.
Billing is built on the trip log. Modivcare requires a signature on every trip log. If the rider cannot sign, a guardian or companion signs on the rider’s behalf, and an unaccompanied rider who cannot sign is marked “UTS.” Logs without a signature may be denied.
Payment runs on check run dates posted in the provider portal. Virginia shows how it works in practice:
- Weekly cycle. In 2026, invoices are due at the claims center by 5 p.m. Eastern on a Wednesday, and the check run falls on the Thursday of the next week. Invoices due September 23, 2026 are paid on October 1. Holiday weeks shift.
- Paper originals. Virginia providers mail the billing invoice with the original daily trip logs. Faxed or emailed logs are not accepted.
- Direct deposit. Setting up EFT takes three to four weeks.
- Denials. The Batch Detail Report lists denied trips. Resubmit with the trip log and a Resubmit Form.

What changed in 2025 and 2026
- August 20, 2025. Modivcare files for Chapter 11. Nasdaq suspends the stock on August 28.
- October 2, 2025. South Carolina’s Chief Procurement Officer grants Modivcare’s protest of the state’s intent to award the NEMT contract to MTM, in part, and sends the procurement back.
- December 29, 2025. Modivcare emerges as a private company with debt cut by more than 85 percent.
- January 2, 2026. Modivcare takes over Arkansas Region G (Pulaski, Faulkner, and Lonoke counties) from Verida.
- January 16, 2026. South Carolina’s Procurement Review Panel issues its order on MTM’s appeal and upholds the protest decision. Modivcare stays on as the state’s broker.
- April 1, 2026. Georgia moves the Central, East, and Southwest regions to Verida.
- July 1, 2026. Modivcare starts scheduling MaineCare rides in the midcoast region (Waldo, Knox, Lincoln, and Sagadahoc counties).
- August 4, 2026. Maine’s highest court upholds the award for Regions 2, 3, 4, and 8 and lifts its stay, which puts Modivcare on track to serve MaineCare members statewide.
- September 1, 2026. Montana’s new reservation, provider, and facility lines open with Modivcare.
- October 1, 2026. Montana’s anticipated start date for rides with Modivcare. The same day, Blue Cross and Blue Shield of Texas moves its Medicaid members to MTM Health.
- November 1, 2026. Blue Cross and Blue Shield of New Mexico moves its members to MTM Health.
Virginia is also in transition. DMAS still lists Modivcare as its fee-for-service broker, while MTM Health trains providers and drivers who plan to run fee-for-service trips under MTM.

Keeping Modivcare trip logs clean
