Who Alivi is
Alivi Health is a Miami company that manages transportation and specialty benefits for health plans. It was founded in 2016. Magdiel Rodriguez is CEO and Caleb Rojas is President. Besides NEMT, Alivi builds networks for physical therapists, optometrists, and musculoskeletal specialists, and it runs a benefits card with spending wallets.
Unlike many NEMT managers, Alivi calls itself a healthcare company. For a transportation provider, that shows up in who it serves: elderly, frail, disabled, and low-income members of Medicaid and Medicare Advantage plans. Alivi manages its benefits from authorization through payment of claims.
In December 2024 Alivi said two new Florida contracts, one with a Medicaid plan and one with a Medicare Advantage plan, would more than double the company and add about 250 jobs in Florida. It said a Colorado-based company had held those contracts before.

Sunshine Health is Alivi’s Medicaid anchor. The Sunshine lines Alivi runs include long-term care, the MMA plan, and the Pathway to Shine, Mindful Pathways, and Power to Thrive specialty plans.
What Sunshine Health trips look like
Sunshine members get unlimited rides to medical appointments when they have no other way to get there. The benefit covers bus, wheelchair van, taxi, and other vehicle types, plus transfers between hospitals or facilities and escorts when medically necessary. Out-of-state trips and any trip over 100 miles need prior authorization, and every trip is coordinated through Alivi.
Rides can be booked by the member, a parent or guardian, a Sunshine case manager, or a medical provider or facility. Alivi books Sunshine rides Monday through Friday, 8 a.m. to 5 p.m. Eastern, and runs real-time ride support 24 hours a day.

How to become an Alivi transportation provider
Alivi’s process starts with a callback request.
- Fill out the callback form. It is on the Join Alivi’s Network as a Transportation Provider page. It asks for your name, company, phone, email, state of service, counties of service, NPI, total drivers, total vehicles, the levels of service you run, and the dispatch platform you use.
- Wait for the callback. Alivi says its team responds within two business days.
- Credential with a provider relations specialist. Expect the standard broker file: insurance certificates, business license, driver records, and vehicle documents. Our credentialing guide lists what brokers usually ask for.
- Contract and set up the portal. Alivi’s transportation provider portal is where you work trips and process payment.

Alivi requirements
Two parts of Alivi’s program set the bar for providers:
- Compliance. Alivi’s compliance program covers fraud, waste, and abuse, with compliance training and a downstream provider oversight policy for the companies it contracts.
- Insurance program eligibility. The Cluett NEMT Insurance Program for Alivi providers requires at least three years of continuous NEMT operations, drivers with at least two years of driving experience, and continuous commercial auto insurance with no lapses or reinstatements for nonpayment.
The insurance program, developed with Blue Crescent Risk Services, uses A-rated admitted carriers. It covers commercial auto liability, physical damage, general liability, professional liability, and loading and unloading liability. It offers premium financing with no deposit, spread over 10 equal monthly payments, with the first payment due 30 days after the policy starts. Participants also join Blue Crescent’s risk management and driver training programs. Coverage is subject to carrier underwriting.
How trips reach you
Alivi assigns trips with auto-assignment technology tailored to a custom profile for each provider. A provider relations specialist supports your account, and Alivi’s Ride Assist team handles problems during a trip. Its contact center runs 24 hours a day.
Members book through apps as well as by phone. Sunshine and Wellcare members use the AliviRide app, and CareTrip is Alivi’s second member app. Facilities book through the EpicRide portal. Every mode is GPS tracked in real time, including rideshare, curb-to-curb, door-to-door, wheelchair, and stretcher.
Service rules come from each plan. On the Wellcare program:
- Members request routine rides at least 72 hours (three business days) ahead, and up to 30 days ahead.
- Will-call returns are activated through Ride Assist, and the provider has up to 60 minutes from the call to pick up. See will-call trips.
How Alivi pays providers
Alivi processes provider claims through its transportation provider platform, the same portal where you manage trips. Rates and payment terms are set in your provider agreement, so confirm the payment schedule and billing deadline before you sign.
Keep pickup and drop-off times, the driver and vehicle, and the member’s signature complete for every leg.
What changed in 2025 and 2026
- December 12, 2024. Alivi announces two new Florida contracts that it says will more than double the company.
- January 1, 2025. Alivi starts Sunshine Health’s Medicaid transportation, replacing Modivcare. Standing orders carry over.
- January 30, 2026. CarePlus awards Alivi its optometry benefit starting April 1, 2026, which widens Alivi’s CarePlus relationship beyond transportation.
