LKLP provider guide: Kentucky Medicaid rides in Regions 5, 13, and 15
Overview
LKLP Community Action Council, a nonprofit in Hazard, holds the Medicaid ride brokerage for 32 Kentucky counties in HSTD Regions 5, 13, and 15. Its own vehicles cover its four home counties, Leslie, Knott, Letcher, and Perry, while outside carriers serve the rest. Who LKLP is
LKLP Community Action Council, Inc. is a 501(c)(3) nonprofit headquartered in Hazard, and its initials stand for the four counties it serves as a community action agency: Leslie, Knott, Letcher, and Perry. Its transportation office at 398 Roy Campbell Drive has two jobs. It puts riders in LKLP’s own vehicles, and it holds the Medicaid broker contract from the Transportation Cabinet for three Human Service Transportation Delivery (HSTD) regions totaling 32 counties.
Transportation is the bulk of LKLP’s money. The 2024-2025 annual report shows $40,188,036 in state funds from the Transportation Cabinet for HSTD and public transit, inside a total agency budget of $62,069,065.
Statewide subcontractor rules do not change from one region to the next: motor carrier authority, the three-driver minimum, Provider Type 56 enrollment, and the broker pay calendar. LKLP’s three regions
LKLP’s three regions sit in two separate parts of the state, and the monthly per-member payment from the Department for Medicaid Services (DMS) differs widely among them.

These capitation figures come from the rate slide DMS and the Cabinet showed the legislature’s budget subcommittee in November 2025. No region was paid more than Region 5, and only Region 11 was paid less than Region 15. An actuary working for DMS sets each region’s rate from that region’s own members and trip history, and CMS approves it. Under House Bill 2 of 2026, the rate must also account for each region’s average trip time and distance, its geography and terrain, and its population density.
Region 13 and Region 15 touch in one place, where Wolfe County borders Menifee and Morgan counties. Region 5, around Bowling Green and Glasgow, touches neither. Its neighbors are Regions 2, 3, 4, 8, and 12, so a Region 5 company looking for more counties would sign a separate subcontract with GRITS for Region 3 or 4, or with RTEC for Region 12. The regions keep different clocks as well. The federal time zone boundary in 49 CFR 71.5 leaves 11 of Region 5’s 12 counties on Central time, with Taylor County the one on Eastern. Every county in Regions 13 and 15 runs on Eastern time. LKLP quotes its hours in local time, so service in Warren County starts at 6 a.m. Central, an hour after the 6 a.m. Eastern start in Perry County.
LKLP’s own fleet beside its subcontractors
LKLP drives as well as brokers, but mostly close to home. Its 2023-2024 annual report describes a program that transports riders within the four home counties, all in Region 13, and subcontracts with carriers for the other 28.
The 2024-2025 report puts numbers on both sides:
- LKLP’s vehicles. 72 vehicles and 48 drivers made 96,440 trips in the four-county area: 73,957 HSTD trips, 14,320 public transit rides, 7,827 Job Access Reverse Commute trips, and 336 for the Department of Corrections.
- The brokerage. A separate figure counts 671,449 brokerage HSTD trips.
Since LKLP also carries riders itself, Kentucky’s rider choice rule matters here. KRS 281.874 lets riders whose certification calls for Type 07 or Type 08 service ask for LKLP or for another eligible subcontractor working the area, and they still make that request through LKLP.
What LKLP asks riders when they book
Every LKLP trip begins with a call to a reservationist, who asks for:
- The rider’s name exactly as printed on the Social Security card, or on the medical card for a Medicaid member.
- The date of travel.
- Full pickup and destination addresses, with suite or building numbers and the doctor’s name.
- The time the rider needs to arrive.
- When to come back, or the arrival time for a second appointment.
- Seats for anyone riding along, such as a personal care attendant, a child, or a guest.
- Special transport needs, for example a lift van.
Non-urgent trips need at least 72 hours’ notice. Reservationists take calls weekdays from 8 to 4:30 and Saturdays from 8 to 1, and vehicles are on the road 6 a.m. to 8 p.m. Monday through Friday plus the same Saturday morning hours. Urgent care requests outside those hours, Sundays and state holidays too, go to a paging service.
Medicaid members are not LKLP’s only riders. In all 32 counties it also books trips for Department for the Blind and Vocational Rehabilitation participants.
How pay and rates work in LKLP’s regions
LKLP receives a monthly payment for each assigned member and pays its subcontractors by the trip. It does not choose the trip rate. The Cabinet sets a single rate per certificate type for each delivery area, and every provider there gets it, LKLP’s own vehicles included.
Each region’s rates can change on their own schedule. The Cabinet’s fiscal 2023 NEMT report records that six regions, Regions 1 through 5 and Region 14, had their subcontractor rates adjusted as of May 1, 2023. Region 5 was part of that change, and Regions 13 and 15 were not. A company planning trips in more than one LKLP region should get the current rate for each. House Bill 2, which became law on April 14, 2026, also has each regional broker equip every vehicle serving Medicaid riders with GPS that lets the broker pinpoint it at all times on those trips, at the broker’s cost. Getting into LKLP’s network
Phone LKLP’s transportation office (606-436-8853) once the Division of Motor Carriers has issued your Taxi or Disabled Persons Vehicle certificate. Ask which regions and which certificate types LKLP is short on, because its three regions are far apart and may pay differently.
The Cabinet’s statewide process takes over from there. LKLP walks through the contract, checks each vehicle, and keeps on file your insurance, each registration, proof of workers’ compensation coverage, your operating certificates, and the fee receipt card issued for every vehicle. It signs your MAP-811 Medicaid applications before you submit them. New providers also need three or more approved drivers, with three vehicles in running order to match.

Keeping LKLP work organized
Under 603 KAR 7:080, the broker or subcontractor certifies to the Cabinet that each driver is licensed for the assigned vehicle and has completed first aid, bloodborne pathogen, and passenger assistance training. - Does LKLP give Medicaid trips to outside transportation companies?
- Yes. LKLP's 2023-2024 annual report says its own program drives riders only inside Leslie, Knott, Letcher, and Perry counties and hands the trips in 28 further counties to subcontracted carriers.
