What is a stair chair in NEMT?
A stair chair is a lightweight, folding chair with handles, and sometimes a tread or track system, that crews use to move a seated rider up or down stairs. NEMT providers use it for riders in walk-up homes who cannot manage steps. Stryker's manual and powered models are rated for 500 pounds, including equipment. There is no national billing code for stair chair service.

What a stair chair is
A stair chair is a portable chair that lets trained staff move a seated person up or down a flight of stairs. It folds flat for storage in the vehicle. Stryker’s Stair-PRO, for example, has wheels for flat ground and lift handles at both ends, with patient restraints and an oxygen bottle holder offered as options.
Some chairs add a track. Stryker’s Stair-PRO uses a tread system that lets caregivers bring a patient down stairs without lifting the chair. Stryker’s XPEDITION is powered and can climb as well as descend.
A stair chair is not the rider’s wheelchair and not a stretcher. It is a transfer device used for the stairs only. At the bottom, the rider moves to their wheelchair, a vehicle seat, or a cot.
When NEMT trips need one
Stair chair service fits riders who live in walk-up homes or apartments and cannot walk the steps, even with help. Typical cases include riders coming home from a hospital stay, frail older riders, and wheelchair users on an upper floor with no elevator.
Programs treat stairs differently:
- Virginia. Drivers do not assist wheelchair riders up or down stairs. For stretcher van riders, the provider must supply a scoop, Reeves, or stair chair stretcher if the rider needs one. Stretcher vans there already run with two crew members.
- Oregon (CareOregon). Drivers guide riders on stairs when asked but may not physically assist or carry them up or down. Drivers must tell the NEMT manager when a rider needs that help.
- New York. Ambulette personal assistance includes physical help climbing or descending stairs.

Stryker’s specification sheet says users must count the weight of the patient, equipment, and accessories against the load rating. A rider close to 500 pounds, or a narrow, steep, or winding staircase, needs a different plan. See bariatric transportation for heavier riders.
Intake questions
Ask these at booking so the right crew shows up:
- How many steps are there, and are there landings or turns?
- How wide is the staircase?
- What does the rider weigh?
- Can the rider sit upright for the whole descent?
- What happens at the bottom: wheelchair, vehicle seat, or stretcher?
How stair chair trips are billed
HCPCS has no stair chair code. The trip bills under its mode, such as T2005 for a stretcher van or A0130 for a wheelchair van. Private-pay work is different. You set the price, so build the second staff member and the extra time into the quote. Our guide on pricing NEMT trips covers add-on fees.

Frequently asked questions
- Can a NEMT driver carry a rider down stairs without a stair chair?
- Usually not. Virginia's handbook says drivers do not assist wheelchair riders up or down stairs, and CareOregon's manual has drivers guide riders on stairs but bars them from physically assisting or carrying a rider up or down. A rider who cannot walk the steps needs stair chair service with trained staff.
- How much weight can a stair chair carry?
- Check the model. Stryker rates both its manual Stair-PRO and its powered XPEDITION at 500 pounds, and that limit includes the patient, equipment, and accessories. Tell the broker when a rider is heavier than the chair's rating.
- Is stair chair service billed separately?
- Usually not. HCPCS has no stair chair code, so the trip bills under its mode, such as T2005 for a stretcher van. On private-pay trips you set the price and can add a stair fee. Ask your broker or health plan whether they pay anything extra before you quote a rider.
