Notice of cancellation endorsement: making your insurer warn a broker or facility before a policy ends
Overview
A notice of cancellation endorsement makes your insurer send written notice to a named broker, county or facility before your policy ends. Without it the notice goes only to you, and an ACORD certificate says it confers no rights. Louisiana, Arizona, Indiana and Ohio buyers ask for 30 days, Transdev in Oregon asks for 15, and Paratransit Services in Washington asks for 45.

Will a certificate of insurance warn the holder?
No. A standard policy sends its notices to you, not to the people you gave a certificate. IRMI says the first named insured receives the cancellation notice and any return premium, and ISO’s general liability form has the insurer mail a nonrenewal notice to the first named insured shown in the declarations, at least 30 days before expiration. The ACORD 25 certificate adds nothing. Its cancellation box only promises that any notice will follow the policy’s own provisions, and the form says it gives the holder no rights.
Texas ties a holder’s right to notice to the policy itself. The holder must be named in the policy or in an endorsement, and the policy, that endorsement or a state rule must call for the notice. The certificate of insurance entry covers the rest of that rule.
How many days of notice do buyers ask for?
From 15 to 45 days, and the wording varies. These requirements show the range.
| Buyer | Notice | What the contract says |
|---|---|---|
| LCP Transportation, Indiana (July 2026 checklist) | 30 days | Wording in the certificate’s description box, for any reason including nonpayment |
| Louisiana Medicaid managed care (March 2026) | 30 days | A 30-day cancellation clause on the policy, issued to the health plan |
| WellTrans, Indiana (October 2025) | 30 days | Policies indicate WellTrans is informed before any termination or change |
| Arizona Medicaid plan subcontracts (January 2025) | 30 days | No expiry, suspension, cancellation or material change without written notice |
| Hamilton County, Ohio (December 2025) | 30 days | Each policy endorsed, with notice to the county risk manager and job and family services |
| Transdev Health Solutions, Oregon (2024 manual) | 15 days | Cancellation, renewal, restriction or nonrenewal of any coverage |
| Paratransit Services, Washington | 45 days | Insurer notice by certified mail before any cancellation, lapse or material change |
| MTM, Pennsylvania agreement (2023) | Not stated | A specific notice of cancellation endorsement, copies at signing and renewal |
Several ask for more than a cancellation. Arizona, Hamilton County, WellTrans and Paratransit Services also cover a material change or other change in coverage, and Transdev covers renewal and restriction.
How do you ask your agent for it?
- Copy the clause from each contract: the days, the name and address to notify, which policies it covers, and whether a material change counts.
- Ask your agent whether the insurer will endorse the policy for that holder, and what it costs, before you sign the contract.
- Ask for the endorsement itself, not only certificate wording. MTM wants copies of its endorsements at signing and at every renewal.
- Have the certificate repeat what the endorsement says. In Texas it may carry a 30-day statement only if the policy has the same one.
- Repeat the request at each renewal, so every holder has current endorsements before the old ones expire.
What if the insurer will not issue it?
An insurer may decline, so ask early. If it does, ask the buyer in writing what it accepts in its place, and keep the answer in your file. Meanwhile, treat yourself as the notice system. Contracts already put that duty on you: Paratransit Services wants written notice within one business day of any cancellation, nonrenewal or termination notice you receive, and WellTrans wants new certificates immediately on any change in coverage. Keep a list of every holder and every expiration date, and call a holder the day an insurer notice arrives.
What the endorsement does not do
It tells the holder; it does not keep the policy alive. The insurer’s own notice periods to you come from state law, and the guide to cancelled and non-renewed policies lists them. A cancelled policy threatens the contract whoever was warned. MTM’s Pennsylvania agreement ends immediately if insurance is not maintained at all times, and a missed installment on a premium finance agreement can cancel a policy.
Tracking the dates
HealthRide’s fleet registry stores each van’s insurance expiration date and reminds you ahead of it, which gives you the start of the expiry list above. Add the holder contacts next to it in your own records.
Frequently asked questions
- Will an ACORD certificate warn the holder 30 days ahead?
- No. The ACORD 25 cancellation box only promises that any notice will follow the policy's own provisions, and the form states that it gives the holder no rights. Notice to a holder has to come from the policy or an endorsement, so ask your agent for the endorsement first and the certificate second.
- Does the notice also cover a change in coverage, not only cancellation?
- That depends on the buyer's wording. Arizona's Medicaid subcontract terms bar expiry, suspension, cancellation or material change without 30 days' written notice. Paratransit Services covers cancellation, lapse or material change, WellTrans covers any termination of or change in coverage, and Transdev's Oregon manual adds renewal and restriction. Copy the buyer's exact words to your agent.
- Who must tell the broker if my policy is cancelled?
- The insurer does, if the policy is endorsed to give that notice. Contracts also put a duty on you. Paratransit Services in Washington requires a subcontractor to give written notice within one business day of receiving notice of cancellation, nonrenewal or other termination of required coverage.
- What if my insurer will not add the endorsement?
- Ask the buyer in writing what it accepts in its place, before you sign, and keep the answer in your file. Meanwhile, forward every insurer notice to each holder the day it arrives and keep a dated list of policy expirations, so you are the warning the endorsement would have been.
