What is a same-day trip in NEMT?
A same-day NEMT trip is a ride requested on the day it is needed, outside the normal advance-booking rule. Programs such as Louisiana, Georgia, and Virginia expect routine rides to be booked 2 to 5 days ahead, then make exceptions for urgent care and hospital discharges. Georgia and Virginia expect urgent rides to arrive within about 3 hours.

Advance-booking rules and their exceptions
A trip is same-day when it is requested inside the program’s normal notice period, usually on the day of service. Each program sets its own notice rule and its own exception for urgent needs:

The three kinds of same-day work
- Hospital discharges. Time-sensitive, with hard deadlines. Louisiana’s rule is in the table above. MTM’s 2026 Virginia provider handbook gives the vehicle three hours from notice that the member is ready, and missing it can bring liquidated damages. - Urgent appointments. Care that must happen today but is not an emergency.
- Rescues. Rides another provider failed to cover. Colorado lets eligible providers take urgent trips scheduled directly by medical facilities, including when a NEMT provider did not pick up a member from an appointment within one hour of the scheduled time.
A will-call return is not a same-day trip. It was booked ahead, and only its pickup time is open.
Fitting same-day trips into a full day
Brokers want providers who say yes. SafeRide’s June 2023 provider guide expects subcontractors to accept 50 percent of same-day bookings. MTM’s 2026 Virginia handbook expects providers to keep enough vehicles and drivers to respond to short-notice assignments, though MTM will not assign an urgent trip without the provider’s approval. CareOregon’s manual says a broker may ask a provider to run a same-day trip that is already late, and the provider will not be penalized for the late pickup when the trip was assigned late.
Saying yes without breaking the schedule takes planning:
- Leave slack in a few routes, especially near hospitals and urgent care clinics.
- Know where every vehicle is and what it can carry, since a discharge may need a wheelchair or stretcher.
- Reuse gaps. A late cancellation opens a slot that a same-day trip can fill.
- Answer fast. CareOregon notes that brokers may ask providers to accept same-day and next-day requests within a much shorter timeframe.

Frequently asked questions
- Is a will-call return a same-day trip?
- No. A will-call is booked in advance as part of a round trip, and only the return pickup time is left open. A same-day trip is a new request that did not exist when the day started, such as a hospital discharge or an urgent care visit.
- What counts as urgent for NEMT?
- Each program defines it. Georgia calls urgent care an unscheduled situation with no immediate threat to life or limb, where the member must be seen that day and treatment cannot wait. Louisiana includes chemotherapy, radiation, dialysis, and opioid treatment that cannot be rescheduled.
- Do I have to accept same-day trips from a broker?
- Brokers let providers accept or reject offered trips, but they watch how often you say yes. SafeRide's June 2023 provider guide expects providers to accept 50 percent of same-day bookings, and its current standards require a 95 percent ride acceptance rate overall. CareOregon treats turning back critical same-day trips you already agreed to accept as an unacceptable reason for reassignment.
