# Add a booking form to your website

> Make a ride request form, then put it on your website or share it as a link or QR code. Every request waits for you to accept it.

With a booking form, riders and families can ask you for a ride any time, without calling. Each request comes to you first. Nothing goes on your schedule until you accept it.

## Before you start

**You'll need**

- An owner or admin sign-in. Only they can open **Booking forms**.

**You can add later**

- A website. Without one, share the form as a link or a QR code.

## Steps

1. At the bottom of the menu on the left, select **Settings**. Under **Operations**, select **Booking forms**.

2. Select **Create booking form**. The **Create booking form** box opens.

3. In **Form name**, type a name only you will see, like **Main website**.

4. If you need to, open **More options**. There you can set the **Minimum notice** riders must give, ask for a date of birth or email, and set a **Button color** to match your website.

5. Select **Create form**. Your form shows on the page with four tabs:

   - **Add to your website**: code for an HTML or custom-code block on your site. The form shows right on the page.
   - **Simple embed (fixed height)**: for website builders that block that code.
   - **Share a link**: a link for emails and texts, and a QR code to print.
   - **Ask AI to add it**: a prompt for an AI website builder's chat.

6. Pick a tab and select **Copy code**, **Copy link** or **Copy prompt**. Paste it into your website, or share the link. Not sure which tab fits? Open **Works with your website builder** at the bottom of the page.

## What riders do

Riders fill in four short pages: **Who's riding**, **What do you need**, **Where and when**, and **Review & send**. They can ask for a ride back, at a set time or as a will-call. After **Send request**, they see "Request sent" and a confirmation code. If they checked **Text me updates about this trip**, they get a text when you answer.

## What you see

The request shows in **Trips**, on the **Requests** tab, marked **Website**. Accept or reject it there: see [Accept or reject trip requests](/knowledge-base/trips/trip-requests/). If the rider is already in **Passengers**, the request goes to them. If not, HealthRide adds them.

## Good to know

- **Riders pay their own way.** Requests from the form bill the rider. If one payer covers every ride, like on a facility's own website, pick it in **More options** under **Bill these requests to one payer instead**.
- **Pause it any time.** Select **Pause new requests** on the form, and **Resume** to turn it back on. The code can stay on your website.
- **Keep it on your own site.** In **More options**, add your website under **Allowed websites**, and no one can copy the form onto another site. Editing the form later doesn't change the code on your website.

## If something looks wrong

| What you see | What to do |
|---|---|
| There's no **Booking forms** in Settings | Only owners and admins can see it. Ask your owner. |
| Riders see "This provider needs at least 2 hours notice." | That's your **Minimum notice**. Lower it in **More options**, or ask them to call you. |
| The form doesn't show on your website | Your website builder may block the code. Use **Simple embed (fixed height)**, or share the link. |
